Hardeman County Community Health Center (HCCHC) is a Non-Profit Federal Qualified Health Center (FQHC) who provides comprehensive, integrated and quality health care services to improve the health and well-being of our patients and communities we serve.

The mission of Hardeman County Community Health Center is to provide quality, accessible, affordable primary health care services to the residents of Hardeman County, Haywood County, Chester County, and neighboring counties.

Hardeman County Community Health Center is striving to operate state-of-the-art professional medical centers with quality providers having all essentials necessary to offer exceptional patient care with maximum accessibility and affordability for patients.

Our Centers

A patient-centered medical home is an approach for providing comprehensive medical care to patients and their families. This system is based on ongoing provider-patient relationship. The provider not only handles basic health care needs, but is also responsible for coordinating care with other qualified providers, offering health education and helping patients to properly manage any chronic conditions. Quality and safety are hallmarks of our health centers. We pledge to offer compassionate, accessible, and comprehensive medical care to all our patients.
